FCT Logic Model – Andrea Pike

Project overview:
This part of our grant is a project to co-create a clear picture of what NL family care teams are meant to do. By this we mean, we want to describe the essential components of FCTs such as the team members, their roles, responsibilities to (and within) the team and their day to day activities along with how those components are meant to lead to collaborative care and how collaborative care is hypothesized to lead to better outcomes within the quintuple aim. This is typically referred to as a logic model.

To do this we are conducting a review of all available and relevant documents that describe NL FCTs. Generally, there are 3 main places to find documents, the intervention developers, the intervention providers and the supporting literature. The first place I begin is with the intervention developer and in out project that is the government and they developed the FCT policy framework which is our primary document for review. This review has been complete and additional documents have been sought. I am in the process of setting up discussion with the relevant health provider groups (NLHS and NLMA) to obtain other documentation and lastly, Brad Furlong is leading the literature review and any identified studies that meet our eligibility criteria will be included for review.

June Progress: In the slide below, I have outlined my document review progress to date. I am currently focused on obtaining the NLHS on-boarding and evaluation documents. I plan to have a completed version to send for review by August in advance of our full team meeting in September where will map the FCT activities to areas where they interact with the health system as a means of identifying any contextual factors that could influence success.

In September, we plan to hold a 1 hour session with government and NLHS and FCT managers. The session has two key objectives:

  • to ensure agreement on the logic model we co-created via document and individual discussions in June.
  • to identify points in the system where the FCT activities interact with contextual factors that could positively or negatively impact completing those activities.

The overall goal is to identify areas in the system that act as barriers or leverage points and include these in the next project in our grant, the systems-based process analysis.
