Project Overview: This part of the FCT-AImS grant is all about assessing “on the ground” and “in the clinic” activities. Within this grant, we only have capacity to assess up to 4 FCT sites. We want to get a complete picture of what is happening within each of the 4 selected teams and how their FCT is similar or different to logic model we co-created in my previous project on the NL FCT-logic model. We want to understand how each provider describes the purpose and goals of the team and their role in the team and how this translates to day-to-day activities to achieve the goal. We want to understand how the providers think this model and way of working is different to the traditional model of primary care and how they think (with examples) this might be producing better outcomes for patients, providers and the system. We want to explore the challenges they are facing in completing their day to day activities and how they have worked (or are still working) together as a team to overcome those barriers. In particular, uncovering and describing any modifications they have made to the planned logic model will help us to reshape the initial thinking about how it is working. Because we haven’t come across any one document outlining exactly how the team should work together and because we know that TBPC is a pragmatic intervention made of multiple components we hypothesize that the day to day activities will be different from one team to another and we want to learn how and why this is happening and help to develop a comprehensive evaluation framework that will capture which activities seem to have the most success on team functioning and resulting outcomes. For any current challenges we uncover, we will co-develop solutions hand-in-hand with the team and any practice facilitation supports (e.g. practice facilitators) already in place. What we can bring to the existing practice facilitation discussions is expertise identifying and choosing strategies with an implementation science and systems thinking approach.
June Project Update: We are planning to start the site evaluation in Fall 2026. Once the initial logic model is finalized we will be able to choose the sites that would be considered the most “operational” for evaluation.
